Course Description

This 2-day course will walk the participants through the process of determining and identifying the most appropriate ICD-10-CM codes.  Participants will actively use the correct process for using the ICD-10-CM Coding Manual, from identification of the main term through the final code selection and review the ICD-10-CM Coding guidelines and chapter-specific guidelines. The Patient-Driven Payment Model (PDPM) has increased the importance of accurate ICD-10-CM diagnoses by focusing payment based on resident characteristics. ICD-10-CM codes and diagnoses will impact the OT, PT, SLP, NTA, and nursing components under PDPM.  State case mix systems are reviewing the PDPM process to determine how the new HIPPS codes can impact state Medicaid payment in the future.

Learning Objectives
  1. State the correct process for determining the ICD-10 code from the physician’s documented diagnoses.
  2. Practice ICD-10 -CM coding using the ICD-10-CM Coding Manual from the identification of the main term and the main subterms in the alphabetic index.  Use the code identified from the alphabetic index to locate the most specific code for the diagnosis in the tabular list based on the highest specificity for the diagnosis documented by the physician in the medical record.
  3. Explain how the 7th character is added when the base code has less than 6 characters.
  4. Report one strategy for determining the primary diagnosis.
Required Material:

Please make sure you have the latest ICD-10-CM coding manual. Your manual needs to have codes that are valid from October 1, 2021 through September 30, 2022.

Carol Maher is a Board Certified Gerontological Registered Nurse with over 30 years long term care experience and has worked in long term care in many roles. She worked as the MDS Coordinator in a large (1,200 bed) facility and in facilities with high Medicare censuses with rapid admission and discharge processes.  Carol has worked as one of the Gold Standard nurses for MDS 3.0, serving on the RAP workgroup to prepare the way for the CAAs for MDS 3.0, and participating on a number of Technical Expert Panels related to MDS, Quality Measures and care planning.

Carol is a Certified Professional Coder (CPC).  She has maintained her certification with continuing education and provides ICD-10-CM training to SNF clients across the country. Ms. Maher has authored and recorded the AANAC certificate class regarding ICD-10-CM coding in long-term care.

Ms. Maher is the Director of Education for Hansen Hunter & Co., providing MDS and Medicare classes across the country, presenting monthly educational webinars and completing compliance audits. She is the author of Long-Term Care MDS Coordinator’s Field Guide (HCPRO 2016).
